Roles & Responsibilities

Provide compassionate direct patient care to residents, ensuring dignity and comfort
Administer medications accurately and document each dose according to policy
Monitor and record vital signs, reporting any changes in condition promptly
Collaborate with nurses, therapists, and care team members to deliver coordinated care
Maintain clear, accurate patient records and follow facility documentation standards
Support residents with activities of daily living and promote a safe environment
Follow infection control protocols and facility policies to protect patients and staff

Qualifications

Active LPN license in Connecticut (required)
Current BLS/CPR certification (required or obtained upon hire)
Completion of an accredited LPN program (diploma or certificate)
Background check and immunizations up to date as required by facility
Strong communication skills and ability to work effectively in a team
Previous experience in long-term care or skilled nursing preferred

Education

LPN diploma, certificate, or equivalent from an accredited program

Knowledge & Skills

Patient assessment and monitoring
Medication administration and documentation best practices
Basic wound care and ADL assistance
Infection control and safety procedures
Electronic medical record (EMR) documentation
Time management, prioritization, and teamwork
