Roles & Responsibilities

• Provide compassionate direct patient care in a skilled nursing setting
• Administer medications and treatments safely and accurately
• Perform routine assessments and report changes in patient condition
• Maintain clear, timely documentation in the patient record
• Collaborate with nurses, therapists and care team members for coordinated care
• Follow infection control, safety and facility policies to protect patients
• Support patients with activities of daily living while promoting dignity and independence

Qualifications

• Current LPN license in Pennsylvania or ability to obtain PA licensure
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable administering medications, wound care and basic treatments
• Strong communication and customer-service skills with patients and families
• Background check and required health screenings (TB, immunizations) as required by facility
• Previous experience in skilled nursing or long-term care preferred but new grads considered

Education

• LPN diploma or vocational nursing certificate
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital signs monitoring
• Electronic health record charting and timely documentation
• Infection prevention, hand hygiene and safety protocols
• Ability to work as part of an interdisciplinary care team
• Good time management and prioritization in a fast-paced environment
