Roles & Responsibilities

Deliver compassionate direct nursing care to assigned patients/residents
Administer medications and treatments accurately and safely
Perform routine assessments and monitor vital signs, reporting changes promptly
Document patient care and observations in the electronic health record clearly
Collaborate with RNs, physicians, and interdisciplinary team members to implement care plans
Provide patient and family education with patience and clarity
Follow infection control protocols and promote a safe care environment
Support admissions, discharges, and transitions of care with professionalism

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania required
Current BLS/CPR certification required
Recent clinical experience as an LPN preferred
Up-to-date immunizations and TB test as required by facility policy
Strong communication skills and ability to work effectively on a care team
Willingness to undergo background checks and meet facility onboarding requirements

Education

LPN / Practical Nursing diploma or certificate from an accredited program

Knowledge & Skills

Medication administration and documentation accuracy
Basic nursing assessments and vital signs monitoring
Familiarity with electronic health records and charting best practices
Infection control principles and safe patient handling
Time management and prioritization in a clinical setting
Patient education, empathy, and effective interpersonal skills
