Roles & Responsibilities

• Provide compassionate direct patient care including bathing, dressing, and personal hygiene support
• Administer medications and treatments according to facility policies and physician orders
• Monitor and record vital signs, patient status changes, and communicate concerns to the care team
• Assist with care planning and participate in interdisciplinary team meetings to support resident outcomes
• Maintain accurate and timely documentation in the resident chart or electronic medical record
• Support a clean, safe, and dignified environment for residents while promoting independence and respect

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• CPR/BLS certification (or willingness to obtain upon hire)
• Current immunizations and TB screening per facility policy
• Ability to pass background checks and required health screenings
• Previous long-term care or skilled nursing experience preferred but new grads with strong clinical skills are welcome

Education

• LPN/Vocational Nursing diploma or equivalent (required)

Knowledge & Skills

• Strong medication administration and documentation skills
• Comfortable with wound care basics and monitoring changes in condition
• Proficient in taking and interpreting vital signs and reporting trends
• Good communication skills for interacting with residents, families, and the care team
• Familiarity with electronic medical records and basic charting practices
• Ability to multitask in a fast-paced long-term care setting with a calm, patient-focused approach
