Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients with skill and warmth
Administer medications and treatments accurately and document thoroughly
Perform routine assessments and report changes in condition to supervising RN or provider
Assist with ADLs (activities of daily living) while promoting independence and dignity
Collaborate with the interdisciplinary team to implement individualized care plans
Maintain clear, timely charting in the electronic health record and participate in shift handoffs
Adhere to infection control practices and facility policies to ensure a safe environment

Qualifications

Current, active LPN license in Pennsylvania (required)
CPR/BLS certification (or willingness to obtain upon hire)
Ability to pass required background checks and health screenings per facility policy
Strong communication skills and a patient-centered attitude
Previous long-term care or skilled nursing experience preferred but not always required

Education

Completion of an accredited Practical Nursing program and possession of LPN license in Pennsylvania

Knowledge & Skills

Medication administration and accurate dosage verification
Basic patient assessment and observation skills
Charting and documentation in an electronic health record
Time management and the ability to prioritize patient needs
Teamwork and effective communication with patients, families, and staff
