Roles & Responsibilities

• Provide skilled physical therapy evaluations and interventions tailored to each patient
• Develop and implement patient-centered treatment plans that promote mobility and independence
• Document progress clearly and timely in the patient record to support continuity of care
• Collaborate with interdisciplinary team members to optimize rehab outcomes
• Educate patients and families on home programs and safety strategies to extend gains beyond the clinic
• Participate in orientation and training to ensure consistent, high-quality care delivery

Qualifications

• Active California PT license (required)
• Graduation from an accredited Physical Therapy program (DPT preferred)
• Current CPR/BLS certification or willingness to obtain
• Clinical experience in skilled nursing, inpatient rehab, or similar settings preferred
• Clean background check and up-to-date immunizations per facility policy
• Excellent communication skills and a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy acceptable with active licensure

Knowledge & Skills

• Patient assessment and goal-setting in post-acute and skilled nursing populations
• Manual therapy, therapeutic exercise, gait training and safe transfer techniques
• Skilled clinical documentation and familiarity with electronic medical records
• Ability to collaborate with nurses, OT, SLP and physicians to coordinate care
• Time management to efficiently deliver high-quality care in shorter shift windows
