Roles & Responsibilities

Provide compassionate direct patient care and assistance with activities of daily living
Monitor and report changes in patient condition to nursing staff in a timely manner
Assist with safe patient mobility and transfers following facility protocols
Document patient care accurately and promptly to support continuity of care
Maintain a clean, safe, and comfortable environment for patients
Collaborate with the healthcare team to deliver person-centered care that preserves dignity

Qualifications

Valid CNA certification in Pennsylvania (required)
Current CPR/BLS certification (preferred or as required by facility)
High school diploma or GED
Strong communication skills and a compassionate, patient-centered approach
Ability to perform physical tasks required for patient care in accordance with facility policy

Education

High school diploma or GED
Current CNA certification (state-specific)

Knowledge & Skills

Hands-on experience with basic patient care and ADLs
Taking and documenting vital signs accurately
Infection control best practices and hygiene standards
Safe patient transfer and mobility techniques
Accurate charting and timely communication with nursing staff
