Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in accordance with care plans
Administer medications and treatments accurately and document thoroughly
Perform timely patient assessments and report changes to the interdisciplinary team
Assist with activities of daily living while promoting resident dignity and independence
Collaborate with nurses, providers, and support staff to deliver coordinated care
Maintain accurate charting and uphold compliance with facility policies and regulatory standards
Support a safe, clean environment and participate in infection control practices

Qualifications

Current, active LPN license for Pennsylvania
CPR/BLS certification (or willingness to obtain upon hire)
Ability to complete required facility onboarding and background checks
Strong communication skills and a team-oriented attitude
Comfortable with medication administration, basic wound care, and documentation

Education

Graduate of an accredited practical nursing program or equivalent vocational training

Knowledge & Skills

Medication administration and monitoring
Basic nursing assessments and vital signs
Charting and electronic health record familiarity
Infection control practices and safe patient handling
Effective communication with residents, families, and clinical team
