Roles & Responsibilities

Provide compassionate direct patient care, including vital signs, wound care, and monitoring
Administer medications accurately and document per facility policy
Collaborate with the interdisciplinary team to create and update patient care plans
Maintain clear, thorough documentation in electronic or paper records
Communicate effectively with patients, families, and staff to promote positive outcomes
Follow infection control and safety protocols to ensure a safe environment

Qualifications

Active LPN license in Pennsylvania (required)
Graduation from an accredited practical nursing program or equivalent
Current CPR/BLS certification or willingness to obtain upon hire
Ability to pass background check and required health screenings
Strong patient-centered communication and teamwork skills
Previous long-term care or relevant clinical experience preferred

Education

Diploma or certificate from an accredited practical nursing program (LPN)

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and infection control principles
Patient assessment and monitoring skills
Familiarity with electronic health records or clinical charting
Strong communication and time-management abilities
Ability to work effectively in a fast-paced healthcare environment
