Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents, focusing on dignity and comfort
• Administer medications and treatments safely and accurately following facility protocols
• Assess residents’ conditions, monitor vital signs, and report changes promptly to the care team
• Document assessments, interventions, and outcomes clearly in the resident record
• Collaborate with RNs, therapists, and support staff to deliver coordinated care plans
• Assist with ADLs (bathing, dressing, mobility) while promoting independence whenever possible
• Support discharge planning and communicate effectively with families and care partners

Qualifications

• Active LPN license in Massachusetts required (RN candidates previously considered per facility needs)
• Current BLS/CPR certification required
• Recent long-term care or rehabilitation experience preferred but new grads with strong skills are welcome
• Excellent communication skills and a team-first attitude
• Ability to work per diem shifts, adapt to changing schedules, and support residents across the care continuum

Education

• Completed accredited LPN (Vocational Nursing) program; equivalent training considered

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Familiarity with wound care, catheter care, and common long-term care procedures
• Comfortable using electronic health records and documenting care accurately
• Ability to prioritize tasks, multi-task in a busy environment, and escalate concerns appropriately
• Person-centered approach with strong interpersonal and problem-solving skills
