Roles & Responsibilities
- Assess residents' condition and respond to changes in health with calm confidence
- Administer medications and treatments accurately and document care thoroughly
- Provide hands-on nursing care that promotes comfort, dignity, and recovery
- Collaborate with the interdisciplinary team to develop and follow individualized care plans
- Educate residents and families on care plans, medications, and discharge goals
- Maintain accurate and timely electronic charting and communicate effectively during shift handoffs
- Support a safe, clean environment through infection control and best practices
Qualifications
- Active LPN or RN license in the appropriate state (state licensure required at time of hire)
- Current BLS/CPR certification required
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Strong clinical assessment, medication administration, and documentation skills
- Reliable, compassionate, and team-oriented with excellent communication
Education
- LPN: Graduate of an accredited practical nursing program
- RN candidates: Graduate of an accredited ADN or BSN program (if applying as an RN)
Knowledge & Skills
- Medication administration and basic IV/ injections knowledge as applicable
- Wound care and pressure injury prevention basics
- Familiarity with electronic health records and accurate charting
- Infection control practices and safe patient handling
- Ability to prioritize, multitask, and respond quickly in a dynamic care setting
- Strong interpersonal skills for working with residents, families, and staff
