Roles & Responsibilities

• Assess residents' condition and respond to changes in health with calm confidence
• Administer medications and treatments accurately and document care thoroughly
• Provide hands-on nursing care that promotes comfort, dignity, and recovery
• Collaborate with the interdisciplinary team to develop and follow individualized care plans
• Educate residents and families on care plans, medications, and discharge goals
• Maintain accurate and timely electronic charting and communicate effectively during shift handoffs
• Support a safe, clean environment through infection control and best practices

Qualifications

• Active LPN or RN license in the appropriate state (state licensure required at time of hire)
• Current BLS/CPR certification required
• Previous experience in long-term care, rehab, or skilled nursing preferred
• Strong clinical assessment, medication administration, and documentation skills
• Reliable, compassionate, and team-oriented with excellent communication

Education

• LPN: Graduate of an accredited practical nursing program
• RN candidates: Graduate of an accredited ADN or BSN program (if applying as an RN)

Knowledge & Skills

• Medication administration and basic IV/ injections knowledge as applicable
• Wound care and pressure injury prevention basics
• Familiarity with electronic health records and accurate charting
• Infection control practices and safe patient handling
• Ability to prioritize, multitask, and respond quickly in a dynamic care setting
• Strong interpersonal skills for working with residents, families, and staff
