Roles & Responsibilities
- Deliver safe, compassionate bedside care to residents, following care plans and physician orders
- Administer medications and document treatments accurately and timely
- Monitor residents’ conditions and communicate changes to the interdisciplinary team
- Assist with activities of daily living while promoting resident dignity and independence
- Support admissions, discharges, and transfers with clear, organized documentation
- Collaborate with nurses, therapists, and support staff to create a positive care environment
- Participate in quality improvement and infection control practices to keep everyone safe
Qualifications
- Active LPN license in Massachusetts (required); RN license also accepted where applicable
- Current CPR/BLS certification
- Minimum clinical experience in long-term care or rehabilitation preferred
- Strong interpersonal skills and the ability to work well in a team
- Ability to pass background checks and meet facility onboarding requirements
Education
- Graduation from an accredited LPN program (required)
- High school diploma or equivalent (minimum)
Knowledge & Skills
- Solid understanding of nursing fundamentals, medication administration, and documentation
- Familiarity with long-term care and rehabilitation workflows
- Good assessment and critical thinking skills to identify changes in resident condition
- Comfortable using electronic health records and other clinical documentation tools
- Compassionate communication with residents, families, and care teams
