Roles & Responsibilities
- Provide compassionate direct patient care to residents in a skilled nursing setting
- Administer medications and treatments accurately and safely
- Monitor and document patient status, changes and responses to care
- Collaborate with the interdisciplinary team to create and follow care plans
- Educate residents and families on care, medications and safety
- Support a positive, person-centered environment that promotes dignity and recovery
Qualifications
- Current and active LPN or RN license in Massachusetts
- Recent experience in long-term care or skilled nursing preferred
- Current BLS/CPR certification preferred
- Demonstrated ability to document accurately and communicate effectively
- Reliable, professional, and committed to quality patient care
Education
- LPN: Practical Nursing diploma or certificate
- RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Knowledge & Skills
- Strong clinical assessment and patient observation skills
- Medication administration and documentation proficiency
- Comfortable with electronic health records and charting
- Excellent communication and teamwork abilities
- Ability to prioritize and manage time in a fast-paced setting
