Roles & Responsibilities
- Implement treatment plans developed by the supervising Physical Therapist to help patients regain mobility
- Provide skilled therapy interventions including therapeutic exercise, gait training, balance activities, and transfer training
- Assist patients recovering from surgeries, strokes, fractures, and other medical conditions with safe, effective care
- Monitor patient progress and promptly report clinical changes to the supervising PT
- Educate patients and caregivers on home exercise programs and safe mobility techniques to support long-term outcomes
- Complete accurate and timely documentation in accordance with facility and state guidelines
- Collaborate with interdisciplinary team members to ensure coordinated, high-quality care
Qualifications
- Active California PTA license required
- Graduate of an accredited Physical Therapist Assistant program
- Current CPR/BLS certification required
- Previous SNF or geriatric experience preferred
- Strong communication and teamwork skills; comfortable collaborating with nurses, therapists, and caregivers
- Ability to work compassionately with the geriatric population and support patient independence
Education
- Graduate of an accredited Physical Therapist Assistant program

- Therapeutic exercise techniques and progressive strengthening methods
- Gait training and safe ambulation strategies
- Balance activities and fall prevention approaches
- Transfer training and safe patient handling
- Accurate clinical documentation and outcome tracking
- Patient and caregiver education for home exercise programs
- Interdisciplinary collaboration and effective communication
- Time management and prioritization in a skilled nursing environment
