Roles & Responsibilities

• Administer medications and treatments safely and accurately according to orders
• Monitor resident status, assess changes, and communicate concerns to the care team
• Perform wound care and skilled nursing tasks with attention to detail
• Document care in the electronic health record clearly and promptly
• Support residents with activities of daily living and encourage independence
• Collaborate with nurses, therapists and support staff to deliver person-centered care
• Follow infection control and facility safety protocols to protect residents and staff
• Provide compassionate resident and family education and emotional support

Qualifications

• Active Massachusetts LPN license (Licensed Practical Nurse) required
• BLS/CPR certification required or obtained shortly after hire
• Prior experience in skilled nursing, rehab or long-term care preferred
• Strong clinical assessment and medication administration skills
• Excellent communication and teamwork skills; resident-focused attitude
• Ability to pass facility onboarding requirements (background check, immunizations) as required

Education

• Graduate of an accredited practical nursing program (LPN) or equivalent
• Continuing education encouraged to maintain licensure and expand skills

Knowledge & Skills

• Proficient in vital sign monitoring and basic clinical assessments
• Skilled in medication administration, documentation, and care planning
• Familiarity with wound care, catheter care, and common rehab nursing tasks
• Comfortable using electronic health records and clear charting
• Strong interpersonal skills for working with residents, families and multidisciplinary teams
• Ability to prioritize tasks in a fast-paced clinical environment
