Roles & Responsibilities
- Administer medications and treatments safely and accurately according to orders
- Monitor resident status, assess changes, and communicate concerns to the care team
- Perform wound care and skilled nursing tasks with attention to detail
- Document care in the electronic health record clearly and promptly
- Support residents with activities of daily living and encourage independence
- Collaborate with nurses, therapists and support staff to deliver person-centered care
- Follow infection control and facility safety protocols to protect residents and staff
- Provide compassionate resident and family education and emotional support
Qualifications
- Active Massachusetts LPN license (Licensed Practical Nurse) required
- BLS/CPR certification required or obtained shortly after hire
- Prior experience in skilled nursing, rehab or long-term care preferred
- Strong clinical assessment and medication administration skills
- Excellent communication and teamwork skills; resident-focused attitude
- Ability to pass facility onboarding requirements (background check, immunizations) as required
Education
- Graduate of an accredited practical nursing program (LPN) or equivalent
- Continuing education encouraged to maintain licensure and expand skills
Knowledge & Skills
- Proficient in vital sign monitoring and basic clinical assessments
- Skilled in medication administration, documentation, and care planning
- Familiarity with wound care, catheter care, and common rehab nursing tasks
- Comfortable using electronic health records and clear charting
- Strong interpersonal skills for working with residents, families and multidisciplinary teams
- Ability to prioritize tasks in a fast-paced clinical environment
