Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and support their daily living needs
• Administer medications and follow physician orders safely and accurately
• Perform thorough resident assessments and document changes in condition
• Collaborate with the interdisciplinary team to develop and update individualized care plans
• Maintain accurate, timely charting in the facility EMR and ensure regulatory compliance
• Promote a safe, clean, and therapeutic environment for residents
• Support and mentor nursing assistants and newer staff to maintain high standards of care

Qualifications

• Current, active LPN license in the state of practice (or RN license if applying as a Registered Nurse)
• Basic Life Support (BLS) certification required or obtained upon hire
• Experience in long-term care or rehabilitation preferred
• Strong clinical judgment, time management, and communication skills
• Ability to pass standard background and health screenings

Education

• Graduate of an accredited LPN program
• For RN applicants: ADN or BSN from an accredited nursing program

Knowledge & Skills

• Skilled in resident assessment and documentation
• Knowledge of medication administration and basic wound care
• Comfortable working within an interdisciplinary team
• Familiarity with infection control practices and safety protocols
• Proficiency with electronic medical records or willingness to learn quickly
• Compassionate bedside manner and good problem-solving skills
