Roles & Responsibilities
- Provide compassionate, resident-centered bedside care with dignity and respect
- Administer medications and treatments accurately and document thoroughly
- Perform routine assessments and communicate changes in condition to the care team
- Assist with activities of daily living and promote residents’ independence
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
- Maintain accurate clinical records in the facility’s documentation system
- Support safe infection control practices and facility policies
Qualifications
- Active LPN license in Massachusetts (LPN role) — license must be in good standing
- Current Basic Life Support (BLS) certification (or willing to obtain)
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Demonstrated ability to communicate clearly with residents, families, and the care team
- Willingness to complete facility-specific orientation, background check, and health screenings
Education
- Graduate of an accredited LPN program (for LPN role)
- For RN openings (if applicable): Graduate of an accredited ADN/BSN program
Knowledge & Skills
- Strong clinical assessment and documentation skills
- Medication administration and safe handling practices
- Familiarity with wound care basics and ADL support
- Comfort working in a fast-paced long-term care/rehab environment
- Teamwork, time management, and problem-solving under evolving conditions
- Basic computer/EMR navigation for charting and care plans
