Roles & Responsibilities
- Provide direct, compassionate nursing care to residents in a skilled nursing and rehabilitation setting
- Administer medications and treatments accurately and document care thoroughly
- Monitor resident condition and communicate changes to the interdisciplinary team promptly
- Assist with activities of daily living while promoting independence and dignity
- Collaborate with therapists and providers to support individualized care plans
- Maintain accurate charts and participate in shift handoffs to ensure continuity of care
- Model excellent bedside manner and build trusting relationships with residents and families
Qualifications
- Active Licensed Practical Nurse (LPN) license (Massachusetts preferred or compact where applicable)
- Basic Life Support (BLS) certification required or willingness to obtain
- Previous experience in long-term care, skilled nursing, or rehab is a plus
- Comfortable administering meds, charting, and working collaboratively on a care team
- Excellent communication skills and a compassionate approach to resident care
Education
- Completion of an accredited LPN/LVN program (or equivalent vocational nursing program)
- High school diploma or GED required
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Familiarity with skilled nursing and rehabilitation care practices
- Proficient documentation and handoff communication
- Ability to prioritize, multitask, and respond to changing resident needs
- Team-oriented mindset with a focus on resident-centered care
