Roles & Responsibilities

• Provide direct, compassionate nursing care to residents in a skilled nursing and rehabilitation setting
• Administer medications and treatments accurately and document care thoroughly
• Monitor resident condition and communicate changes to the interdisciplinary team promptly
• Assist with activities of daily living while promoting independence and dignity
• Collaborate with therapists and providers to support individualized care plans
• Maintain accurate charts and participate in shift handoffs to ensure continuity of care
• Model excellent bedside manner and build trusting relationships with residents and families

Qualifications

• Active Licensed Practical Nurse (LPN) license (Massachusetts preferred or compact where applicable)
• Basic Life Support (BLS) certification required or willingness to obtain
• Previous experience in long-term care, skilled nursing, or rehab is a plus
• Comfortable administering meds, charting, and working collaboratively on a care team
• Excellent communication skills and a compassionate approach to resident care

Education

• Completion of an accredited LPN/LVN program (or equivalent vocational nursing program)
• High school diploma or GED required

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Familiarity with skilled nursing and rehabilitation care practices
• Proficient documentation and handoff communication
• Ability to prioritize, multitask, and respond to changing resident needs
• Team-oriented mindset with a focus on resident-centered care
