Roles & Responsibilities
- Provide compassionate bedside nursing care to residents in a long-term/rehab setting
- Administer medications and treatments accurately and document per policy
- Monitor and record vital signs, reporting changes in condition promptly
- Support residents with activities of daily living while promoting independence
- Collaborate with the interdisciplinary team to develop and follow care plans
- Maintain accurate, timely charting in the resident record
- Ensure a safe, clean environment and adhere to infection prevention protocols
- Advocate for residents' needs and communicate effectively with families
Qualifications
- Current LPN license (Licensed Practical Nurse) in Massachusetts or eligible for state licensure
- Basic Life Support (BLS) / CPR certification required or obtained upon hire
- Experience in long-term care, skilled nursing, or rehabilitation preferred
- Strong interpersonal skills and the ability to work effectively on a care team
- Reliable, professional, and committed to resident-focused care
Education
- Graduate of an accredited LPN program
- High school diploma or GED required
Knowledge & Skills
- Medication administration and documentation accuracy
- Resident assessment and recognizing changes in condition
- Familiarity with electronic medical records and charting best practices
- Infection control, safe patient handling, and falls prevention
- Clear communication with residents, families, and clinical team members
- Time management and prioritization in a fast-paced clinical environment
