Roles & Responsibilities
- Provide compassionate, safe direct patient care to residents in a skilled nursing/rehab setting
- Administer medications and treatments per orders and facility policies
- Assess and document patient status, changes, and care plans in the electronic chart
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
- Assist with admissions, discharges, and timely patient transfers
- Maintain infection control practices and ensure a safe care environment
- Communicate with residents and families with empathy and professionalism
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing
- Previous experience in long-term care, skilled nursing, or rehabilitation is a plus
- Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Ability to work evening per-diem shifts (3:00 PM - 11:00 PM) and be flexible with scheduling
- Strong communication skills and a team-oriented mindset
Education
- Graduate of an accredited Practical Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Skilled in medication administration and resident assessments
- Comfortable with charting and electronic health records
- Experience with wound care and basic restorative techniques
- Excellent time management and prioritization in a busy environment
- Effective interpersonal skills for working with residents, families, and the care team
