Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments safely and accurately according to orders
• Perform focused assessments and document resident conditions clearly in the chart
• Collaborate with interdisciplinary team members to create and update individualized care plans
• Monitor and report changes in resident status promptly to ensure timely interventions
• Support residents with activities of daily living while promoting independence and dignity
• Follow infection control policies and maintain a safe, clean care environment

Qualifications

• Current LPN or RN license in Massachusetts (required)
• Basic Life Support (BLS) certification or willingness to obtain on hire
• Previous experience in long-term care or rehab nursing preferred but not required for strong candidates
• Ability to pass facility background check and meet health screening requirements
• Dependable, patient-focused, and able to communicate effectively with residents and team members

Education

• LPN: Completion of an accredited LPN/LVN program
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

Knowledge & Skills

• Medication administration and accurate documentation
• Resident assessment, vital signs, and basic wound care
• Familiarity with electronic health records and charting best practices
• Strong communication and teamwork skills
• Knowledge of infection control principles and safe patient handling
