Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term care setting
- Administer medications and treatments accurately and safely
- Perform routine assessments and report changes in condition promptly
- Collaborate with the interdisciplinary team to create and follow individualized care plans
- Document care activities clearly and timely in the resident chart/EMR
- Support residents with activities of daily living while promoting dignity and independence
- Participate in shift handoffs and communicate vital information to the next caregiver
Qualifications
- Active LPN license in Massachusetts (required)
- Current BLS/CPR certification (or willingness to obtain)
- Comfortable working in long-term care/rehabilitation environments
- Strong communication and teamwork skills; resident-focused mindset
- Ability to pass facility background checks and health screenings
Education
- Graduate of an accredited Practical Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Medication administration and documentation best practices
- Basic clinical assessment and vital signs monitoring
- Wound care and ADL support in a rehab/long-term care setting
- Familiarity with electronic medical records or willingness to learn
- Time management and prioritization during busy shifts
- Professional, compassionate resident and family communication
