Roles & Responsibilities

• Provide compassionate direct patient care as a Licensed Practical Nurse
• Administer medications and treatments accurately and safely
• Perform routine patient assessments and document changes in condition
• Assist with activities of daily living and promote patient comfort
• Collaborate with the interdisciplinary team to coordinate care plans
• Maintain clear, timely documentation in the medical record
• Support safe handoffs and communication during shift changes

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing (NH preferred)
• Completion of an accredited LPN / practical nursing program
• Comfortable working nights (7:00 PM - 7:30 AM) and every-other-weekend
• Local candidates only (LOCAL ONLY noted in order)
• Willingness to work occasional overtime (OT not guaranteed)
• Professional, dependable, and able to work collaboratively in a clinical team

Education

• Graduate of an accredited Practical Nursing / LPN program
• Active LPN license required

Knowledge & Skills

• Strong fundamentals in medication administration and patient monitoring
• Skilled in taking and documenting vital signs and basic assessments
• Experience with wound care, ADL support, and basic clinical procedures
• Clear verbal and written communication for effective care coordination
• Familiarity with electronic medical records and accurate charting
• Ability to prioritize tasks in a fast-paced clinical environment
