Roles & Responsibilities

• Administer medications and treatments safely and accurately according to physician orders
• Monitor vital signs and clinical status, promptly reporting changes to the care team
• Provide skilled nursing care focused on rehabilitation and patient mobility
• Assist patients with activities of daily living while promoting independence
• Document assessments, interventions and patient progress clearly in the chart
• Collaborate with RNs, therapists and physicians to develop and execute care plans
• Support a positive patient experience by offering compassionate, person-centered care
• Adhere to facility policies, infection-control practices and safety protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license (eligible to practice in Vermont)
• Valid BLS (Basic Life Support) certification or ability to obtain prior to start
• Experience in rehab/long-term care or related clinical setting preferred
• Availability to work the scheduled shifts (Mon–Thu and Sat evenings)
• Reliable transportation (required) and a professional, flexible attitude
• Ability to pass facility credentialing and background checks

Education

• Practical Nursing diploma or equivalent and successful completion of an accredited LPN program
• Current LPN license required

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Familiarity with rehab-focused nursing care and ADL support
• Comfortable using electronic health records and charting systems
• Strong communication skills for patient/family interactions and team handoffs
• Good time management and ability to prioritize in a busy clinical setting
• Basic wound care, infection control and monitoring for clinical changes
