Roles & Responsibilities

• Conduct thorough patient evaluations to identify functional challenges and goals
• Develop and implement individualized occupational therapy treatment plans
• Use therapeutic activities and adaptive equipment to improve independence in daily living
• Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
• Document patient progress and update treatment plans with clear, timely notes
• Educate patients and families on strategies, home programs, and adaptive techniques
• Advocate for patient needs to ensure safe discharge planning and community reintegration

Qualifications

• Licensed Occupational Therapist in California or eligible to obtain California licensure
• Graduation from an accredited occupational therapy program (OT, OTD, MOT) is required
• Current basic life support (BLS/CPR) certification preferred
• Experience in post-acute care, inpatient rehab, or skilled nursing settings is a strong plus
• Excellent communication skills and a collaborative mindset
• Active professional liability insurance or ability to obtain if required by facility

Education

• Degree in Occupational Therapy (OTR, MOT, or OTD) from an accredited program
• Additional certifications or continuing education in rehab or geriatric OT are a plus

Knowledge & Skills

• Skilled in patient assessment, treatment planning, and ADL training
• Knowledge of adaptive equipment, splinting basics, and therapeutic modalities
• Proficient documentation skills and familiarity with electronic medical records
• Strong clinical reasoning and the ability to individualize interventions
• Comfortable working as part of a multidisciplinary rehab team
