Roles & Responsibilities

• Provide compassionate direct patient care to residents, focusing on comfort and dignity
• Administer medications and treatments accurately and timely per provider orders
• Monitor and document vital signs, patient status, and changes in condition
• Collaborate with the multidisciplinary team to support individualized care plans
• Assist with admissions, discharges, and safe patient transfers
• Maintain clear, professional communication with families and team members
• Follow facility policies, safety protocols, and infection control standards

Qualifications

• Valid Licensed Practical Nurse (LPN) credential (LPN) — required
• Travel LPN (T-LPN) experience or willingness to work in a travel assignment — preferred
• Current, unrestricted license to practice in the state of assignment (WV) or ability to obtain
• Comfort with 12-hour day shift schedule and providing nursing care in a long-term care/skilled nursing setting

Education

• LPN diploma or certificate or equivalent practical nursing education

Knowledge & Skills

• Medication administration and basic clinical nursing skills
• Strong patient assessment and observation abilities
• Clear documentation and charting skills
• Good communication and teamwork in a fast-paced care environment
• Ability to follow care plans and facility protocols
