Roles & Responsibilities

• Assess patients’ physical function and mobility to create personalized treatment plans
• Develop and implement therapeutic exercise and mobility programs to improve outcomes
• Document patient progress and treatment notes accurately and timely in the medical record
• Collaborate with physicians, nurses and rehabilitation staff to coordinate care
• Educate patients and families on home programs, safety and functional goals
• Monitor outcomes and modify treatment plans based on clinical progress
• Promote a safe, therapeutic environment and support discharge planning

Qualifications

• Licensed Physical Therapist in California (or eligible to obtain state licensure)
• Graduation from an accredited physical therapy program (DPT preferred)
• Current Basic Life Support (BLS) certification or willingness to obtain
• Experience in inpatient rehabilitation or post-acute care is highly desirable
• Strong documentation skills and comfort using electronic medical records
• Excellent communication and interdisciplinary collaboration skills

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy acceptable from an accredited program

Knowledge & Skills

• Skilled in therapeutic exercise, gait training and mobility techniques
• Proficient with patient assessment, treatment planning and goal setting
• Knowledgeable about assistive devices, transfers and safe patient handling
• Good clinical reasoning and outcome measurement interpretation
• Comfortable working in a fast-paced contract environment and adapting to new teams
