Roles & Responsibilities

• Provide compassionate bedside care and support to patients, ensuring dignity and comfort
• Administer medications and treatments safely and accurately following care plans
• Monitor and record vital signs, report changes promptly to the care team
• Document patient care and charting clearly in the electronic medical record
• Assist patients with activities of daily living and support their mobility and safety
• Collaborate with RNs, physicians, and interdisciplinary team members to deliver coordinated care
• Participate in patient education and discharge planning to promote positive outcomes

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania required
• Current BLS/CPR certification or willingness to obtain upon hire
• Strong clinical judgment, attention to detail, and dependable work ethic
• Comfortable working in a fast-paced care setting and communicating with patients and families
• Previous LPN experience in long-term care, skilled nursing, or acute care is a plus

Education

• Completion of an accredited Licensed Practical Nurse program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment skills including vitals and basic condition monitoring
• Familiarity with infection control and safety protocols
• Basic wound care and assistance with ADLs
• Effective communication and teamwork within a clinical setting
• Comfort using electronic medical records and charting systems
