Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to adults and/or geriatric patients in the rehab/health area
• Administer medications and treatments accurately and document thoroughly
• Monitor patient status, identify changes, and escalate concerns to the care team
• Collaborate with interdisciplinary team members to support discharge planning and rehabilitation goals
• Maintain clear, timely charting and follow facility policies and infection control practices
• Support patients and families with education, emotional support, and clear communication

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) credential
• Previous clinical experience preferred (LPN/RN) — facility experience a plus
• Ability to work 12+ hour day shifts and handle typical clinical demands
• Strong communication and teamwork skills; dependable and professional

Education

• LPN: Practical Nursing diploma or certificate
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) — per facility preference

Knowledge & Skills

• Skilled in medication administration and basic clinical assessments
• Comfortable with documentation in electronic health records
• Knowledge of rehab/healthcare patient care standards and safety protocols
• Excellent interpersonal skills and the ability to work collaboratively
