Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients in the rehab/health area
- Administer medications and treatments safely and in accordance with facility policy
- Monitor and document patient conditions, vital signs, and progress notes
- Collaborate with the interdisciplinary care team to create and update care plans
- Assist with rehabilitation activities and support patients’ functional goals
- Educate patients and families on care plans, medications, and follow-up needs
- Maintain accurate and timely electronic medical records and handoffs
Qualifications
- Current LPN license in New Mexico (required)
- Basic Life Support (BLS/CPR) certification (required or obtain upon hire)
- Experience in rehab, long-term care, or acute care preferred
- Strong patient communication and teamwork skills
- Up-to-date immunizations and ability to pass required health screenings
Education
- Graduate of an accredited LPN program or equivalent vocational nursing training
Knowledge & Skills
- Patient assessment and bedside care
- Medication administration and documentation
- Familiarity with electronic health record systems
- Infection control and safety practices
- Time management and prioritization in a fast-paced setting
- Clear communication with patients, families, and clinical teams
