Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to assigned residents and patients
• Administer medications and treatments accurately and document according to facility policy
• Perform thorough assessments and monitor vital signs to detect changes in condition
• Collaborate with physicians and the interdisciplinary team to develop and update care plans
• Maintain timely, clear clinical documentation in the electronic health record
• Educate patients and families about care, medications and discharge instructions
• Support safe patient transfers, admissions and discharges with attention to continuity of care
• Adhere to infection prevention protocols and promote a safe care environment

Qualifications

• Current, active LPN or RN license in New Mexico or eligibility for licensure
• Recent clinical experience in a relevant acute or rehabilitative care setting preferred
• Basic Life Support (BLS) certification required; advanced certifications welcomed
• Strong communication skills and ability to work effectively as part of a multidisciplinary team
• Proof of immunizations and any required facility-specific clearances

Education

• Graduate of an accredited Practical Nursing program (LPN) or Associate/Bachelor of Science in Nursing (RN)

Knowledge & Skills

• Solid knowledge of medication administration and nursing best practices
• Comfortable with patient assessment, documentation and electronic health record systems
• Able to prioritize care in a fast-paced environment while maintaining attention to detail
• Familiarity with infection control standards and safe patient handling techniques
• Team-oriented with strong clinical judgement and problem-solving skills
