Roles & Responsibilities

• Provide compassionate hands-on nursing care to patients in the Uptown Health/Rehab area
• Administer medications and treatments accurately and on schedule
• Monitor patient status and report changes to the interdisciplinary team
• Document assessments, interventions, and patient responses in the medical record
• Collaborate with RNs, therapists, and physicians to coordinate patient-centered care
• Support patients and families with education, discharge planning, and emotional care

Qualifications

• Active LPN license in good standing (LPN required; RN candidates considered)
• Current BLS/CPR certification required
• Prior experience in a long-term care, rehab, or acute care setting preferred
• Strong clinical assessment skills and medication administration competency
• Excellent communication and teamwork skills, with a patient-first mindset

Education

• Practical Nursing diploma or equivalent (LPN)
• ADN or BSN if applying as a Registered Nurse

Knowledge & Skills

• Skilled in medication administration and wound care
• Proficient with electronic medical records and documentation best practices
• Knowledge of basic rehab nursing principles and mobility support
• Ability to prioritize tasks, manage time during 12-hour shifts, and escalate concerns appropriately
• Strong interpersonal skills for patient and family education
