Roles & Responsibilities

• Provide compassionate, high-quality nursing care to patients in the Uptown Health/Rehab area
• Administer medications and treatments safely and accurately
• Perform thorough patient assessments and monitor changes in condition
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Document patient care and progress clearly and timely in the electronic medical record
• Assist with rehabilitation activities and support patients’ goals for independence
• Educate patients and families on care plans, medications, and discharge instructions
• Maintain infection control practices and model professional standards of care

Qualifications

• Active LPN or RN license in good standing (eligible for New Mexico licensure preferred)
• Current BLS/CPR certification required; ACLS or other certifications are a plus
• Previous experience in med-surg, rehab, or long-term care settings preferred
• Ability to work 12+ hour day shifts and provide consistent, reliable coverage
• Strong communication skills and a team-first attitude
• Up-to-date immunizations and the ability to complete facility onboarding quickly

Education

• Licensed Practical Nurse: Vocational diploma or equivalent
• Registered Nurse: Associate Degree in Nursing (ADN) required; BSN preferred

Knowledge & Skills

• Patient assessment and focused clinical observation skills
• Safe medication administration and documentation practices
• Familiarity with electronic medical record systems and accurate charting
• Ability to collaborate with therapists and ancillary staff in a rehab setting
• Skilled in wound care basics, mobility assistance, and ADL support
• Strong time management, prioritization, and critical thinking abilities
