Roles & Responsibilities

• Provide compassionate bedside care and support to patients in a rehabilitation/health setting
• Administer medications and treatments safely, following physician orders and facility protocols
• Perform patient assessments, monitor vital signs, and report changes promptly
• Document patient care accurately in the electronic medical record
• Collaborate with the interdisciplinary team to create and follow individualized care plans
• Assist with activities of daily living and promote patient independence and comfort
• Participate in shift handoffs and communicate patient needs clearly to incoming staff

Qualifications

• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing
• Active Basic Life Support (BLS) certification; advanced certifications (ACLS) a plus
• Experience in nursing or rehabilitation care preferred; new grads with strong clinical skills are welcome to apply
• Strong clinical judgment, excellent communication, and teamwork skills
• Ability to follow care plans, document accurately, and work efficiently in a fast-paced environment
• Compliance with facility policies, immunization and background check requirements

Education

• Graduate of an accredited Practical Nursing (LPN) or Nursing program
• High school diploma or GED required

• Patient assessment and monitoring

• Wound care basics and pressure injury prevention
• Electronic medical record (EMR) documentation
• Infection control and standard precautions
• Clear communication with patients, families, and clinical teams
• Time management and prioritization during longer shifts
