Roles & Responsibilities

• Provide skilled PTA care in patients' homes, delivering safe and effective orthopedic and neurologic rehabilitation
• Design and implement mobility and strengthening programs using devices like crutches, walkers, and prosthetics
• Perform thorough assessments and deliver individualized treatment plans that improve patient function and independence
• Document visits promptly and accurately using EPIC (remote client), Rover, and OASIS to ensure continuity of care
• Manage an efficient visit schedule (may see up to 6 patients/day) while maintaining high-quality, patient-centered care
• Communicate clearly with interdisciplinary team members, families, and caregivers to support home-based recovery
• Educate patients and caregivers on safe transfers, home exercise programs, and fall prevention strategies

Qualifications

• Current OR or WA PTA licensure required (pending licenses accepted)
• BCLS (Basic Cardiac Life Support) certification required
• Minimum 1 year of skilled Home Health experience required
• Reliable personal transportation and willingness to drive up to ~60 miles some days
• Comfort charting in EPIC, Rover and completing OASIS documentation
• Professional, business-casual appearance (scrubs allowed but not required)

Knowledge & Skills

• Home Health care delivery and visit-based productivity expectations (up to 6 patients/day)
• Charting systems: EPIC (remote), Rover app, and OASIS documentation
• Orthopedic rehabilitation: total hip/knee replacement, post-op shoulder/ spinal procedures, hip fractures
• Neurologic rehabilitation: post-stroke, spinal cord injury, TBI, Parkinson’s, MS, muscular dystrophy
• Safe use and training with mobility devices: crutches, standard/rolling walkers, gait belts, prosthetics, shoulder slings
• Patient education, transfer training, and home safety assessments
