Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments in patients' homes, tailoring plans to individual goals
• Develop and implement measurable care plans and document progress clearly in the medical record
• Educate patients and caregivers on safe mobility, home exercise programs, and fall prevention
• Coordinate care with nursing and interdisciplinary team members to ensure continuity and quality
• Perform required competency testing and skills checklists to maintain clinical standards
• Complete timely documentation and required health screenings to meet agency and regulatory expectations

Qualifications

• Licensed Physical Therapist with verification of license/certification required
• Current BLS certification (listed as required)
• Comfortable with or willing to learn Epic documentation (preferred, not required)
• Able to complete Trinity specialty competency testing and skills checklists
• Must meet personal health requirements including fit test, drug screen, immunizations, and physical/health statement

Knowledge & Skills

• Home health clinical expertise and safe patient handling techniques
• Strong assessment and treatment planning skills for functional mobility and ADL improvement
• Excellent patient and caregiver education skills
• Familiarity with electronic health records; Epic experience is a plus
• Good communication and coordination skills for working with an interdisciplinary team
