Roles & Responsibilities

• Perform comprehensive SLP evaluations for skilled nursing residents
• Provide daily, evidence-based treatment sessions to help residents regain function
• Manage timely and safe discharges with clear patient and caregiver education
• Document care accurately and consistently following CRG policies and procedures
• Collaborate with the therapy team and facility staff to coordinate resident care
• Meet productivity expectations and contribute to a high-performing therapy program

Qualifications

• Active SLP credential/license appropriate for practice (state requirements apply)
• 1-3 years of clinical experience (as listed on the order)
• Prior Skilled Nursing Facility (SNF) experience strongly preferred
• Comfortable working on-site and following facility dress code (black scrubs)
• Ability to meet productivity expectations (90%) and follow CRG P&P
• Strong communication skills and teamwork mindset

Knowledge & Skills

• Skilled in SLP evaluations, treatment planning, and progress documentation
• Experience delivering daily treatments in a skilled nursing environment
• Knowledge of SNF workflows, discharge planning, and interdisciplinary collaboration
• Ability to manage caseloads efficiently and meet productivity targets
• Good clinical judgement, time management, and resident-centered care skills
• Familiarity with documentation standards and facility policy compliance
