Roles & Responsibilities

• Provide skilled physical therapy services to residents in a Skilled Nursing Facility setting
• Assess, plan, implement, and document individualized treatment plans in line with facility and CRG policies
• Collaborate with the interdisciplinary team to support resident progress and safe transitions
• Maintain professional appearance and wear required black scrubs while on duty
• Meet productivity expectations (90% productivity requirement) and contribute to team goals
• Attend interview and coordination with Regional Director of Therapy as requested

Qualifications

• PT or DPT (listed as required job skills: PT, DPT)
• 1-3 years of experience (as stated in job details)
• Ability to work on-site at the facility and adhere to facility/CRG policies
• Willingness to meet productivity expectations and facility attendance requirements
• Comfort working in a Skilled Nursing Facility (SNF) practice setting

Knowledge & Skills

• Familiarity with Skilled Nursing Facility (SNF) practices and resident care
• Strong documentation and clinical reasoning skills
• Ability to work collaboratively within an interdisciplinary therapy team
• Productivity awareness and time management in a rehab setting
• Professional communication with staff, residents, and leadership
