Roles & Responsibilities

• Evaluate residents’ speech, language, cognitive-communication, and swallowing needs and develop individualized treatment plans
• Deliver skilled SLP therapy in a Skilled Nursing Facility setting with compassion and clinical precision
• Document progress and maintain accurate clinical records consistent with CRG policies and facility requirements
• Collaborate with nurses, PT/OT, and interdisciplinary team members to coordinate care and maximize resident outcomes
• Meet productivity expectations and manage caseload efficiently (therapist productivity requirement: 90%)
• Be present on-site to receive guaranteed hours and participate in team meetings and care planning
• Adhere to facility dress code (black scrubs) and follow Continuum Rehabilitation Group policies and procedures

Qualifications

• Active SLP credential as required by facility/state (confirm state licensure for New Mexico)
• 1-3 years of clinical experience (as listed in the job order)
• Experience working in a Skilled Nursing Facility (SNF) or similar post-acute setting preferred
• Strong documentation skills and ability to meet productivity expectations
• Reliable onsite presence and ability to follow CRG policies & procedures
• Willingness to commit to a 13-week contract and acknowledge the 2-week cancellation policy

Knowledge & Skills

• Clinical expertise in speech-language pathology relevant to SNF populations
• Skilled in assessment and intervention for dysphagia, cognitive-communication, and language disorders
• Efficient caseload management and productivity-focused practice
• Clear communication and teamwork within interdisciplinary care teams
• Familiarity with SNF documentation, compliance, and therapy workflows
