Roles & Responsibilities

• Provide compassionate direct patient care and support daily living needs
• Administer medications and treatments accurately and safely
• Perform timely patient assessments and report changes to the care team
• Document care clearly and thoroughly in the patient record
• Collaborate with nurses, providers and support staff to coordinate care
• Assist with admissions, transfers and discharge tasks as needed
• Promote a safe environment and follow infection control best practices
• Support residents and families with clear communication and empathy

Qualifications

• Current LPN license in good standing, valid for practice in New Hampshire
• Completion of an accredited practical nursing or vocational program
• Current BLS or CPR certification, or willingness to obtain upon hire
• Local candidates only as this is a contract local position
• Comfort working the 3:00 PM to 11:30 PM evening shift
• Strong communication skills, attention to detail and ability to work in a team
• Ability to pass background check and drug screen as required by facility

Education

• Graduated from an accredited LPN / Practical Nursing program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and vital signs assessment
• Familiarity with electronic health records and charting
• Time management and prioritization in a fast-paced clinical setting
• Strong patient advocacy and compassionate bedside manner
• Team collaboration and effective handoff communication
