Roles & Responsibilities

• Provide compassionate patient care and assist with daily living activities
• Administer medications and treatments per plan of care and facility policy
• Monitor and document vital signs and patient status accurately
• Communicate clearly with patients, families, and interdisciplinary team members
• Assist with wound care, basic procedures, and patient mobility as directed
• Participate in care planning and update nursing documentation in the EMR
• Support a safe, clean environment and follow infection control protocols
• Actively engage in teamwork to ensure smooth shift handoffs and continuity of care

Qualifications

• Current LPN license (practical nurse) and in good standing (PA licensure preferred)
• Basic Life Support (BLS/CPR) certification or willingness to obtain
• Ability to pass background check and drug screening as required
• Strong communication skills and a patient-first attitude
• Comfortable with electronic medical records and accurate documentation

Education

• Completion of an accredited Practical Nursing / LPN / Vocational Nursing program

Knowledge & Skills

• Solid clinical knowledge of nursing fundamentals and patient assessment
• Skilled in medication administration and monitoring for side effects
• Proficient at taking vital signs and recognizing changes in condition
• Good time management and prioritization during busy shifts
• Collaborative teamwork and clear communication with care teams
• Basic familiarity with infection control and safety procedures
