Roles & Responsibilities
- Deliver compassionate, hands-on patient care following facility policies
- Administer medications and treatments accurately and safely
- Monitor and document patient conditions, reporting changes promptly
- Collaborate with nurses, aides and care team to coordinate patient plans
- Assist with activities of daily living and ensure patient comfort
- Support quality improvement and infection-control practices
Qualifications
- Active LPN (Licensed Practical Nurse) credential — required
- Current basic life support (BLS/CPR) certification — preferred
- Comfortable providing direct patient care in a clinical/skilled setting
- Strong communication skills and ability to work as part of a team
Knowledge & Skills
- Patient assessment and basic clinical skills
- Medication administration and documentation
- Electronic and paper charting best practices
- Infection prevention and safety procedures
- Clear, professional communication with patients and colleagues
