Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and document thoroughly
Perform regular patient assessments and report changes to the care team
Support care planning and coordinate with nurses and clinicians for continuity
Maintain clear, respectful communication with residents, families, and staff
Follow facility protocols for safety, infection control, and documentation

Qualifications

Current LPN (Licensed Practical Nurse) license valid in Pennsylvania
BLS/CPR certification or willingness to obtain upon hire
Up-to-date immunizations and ability to pass background/health screenings
Strong interpersonal skills and a patient-centered approach
Comfortable with electronic health record documentation

Education

LPN diploma or vocational nursing certificate and required state licensure

Knowledge & Skills

Medication administration and basic IV/clinical skills relevant to LPN scope
Wound care, vital signs, and standard nursing assessments
Familiarity with long-term care or skilled nursing workflows is a plus
Good charting practices and attention to detail
Ability to prioritize tasks and work effectively as part of a team
