Roles & Responsibilities

Provide compassionate direct patient care to residents, supporting their daily living needs
Administer medications and treatments accurately and according to physician orders
Monitor, assess, and document patient conditions and changes in a timely manner
Collaborate with the interdisciplinary care team to deliver coordinated, person-centered care
Promote a safe environment by following infection control and safety protocols
Assist with admissions, discharges, and smooth transitions of care

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania (or eligible to obtain)
Current BLS/CPR certification (or willing to obtain upon hire)
Comfortable with medication administration, wound care basics, and documentation
Strong communication skills and a team-focused attitude
Ability to pass background checks and required facility screenings

Education

Completion of an accredited LPN/Vocational Nursing program or equivalent

Knowledge & Skills

Clinical nursing fundamentals for long-term care
Medication administration and documentation best practices
Basic wound care and infection control procedures
Electronic charting and nursing documentation
Effective teamwork, resident-focused communication, and time management
