Contract Type: Per-Diem

Shift Type: 8-hour shift (exact start/end times and days to be confirmed with facility)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate bedside care to residents, ensuring dignity and comfort
Administer medications and treatments accurately and timely
Monitor resident condition and report changes to the care team promptly
Document care activities clearly and professionally in patient records
Collaborate with nurses, therapists, and family members to create person-centered care plans
Assist with activities of daily living while promoting independence and safety
Support onboarding and mentoring of new staff when needed

Qualifications

Active LPN license in Pennsylvania or eligibility to obtain PA licensure
Current Basic Life Support (BLS/CPR) certification
Experience in long term care, skilled nursing, or similar settings preferred
Strong clinical assessment skills and good judgment
Excellent communication and teamwork skills with a patient-centered attitude

Education

LPN diploma or certificate from an accredited program
High school diploma or GED required

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and infection control procedures
Familiarity with electronic health records and charting
Ability to prioritize tasks in a fast-paced care environment
Compassionate resident engagement and family communication
