Roles & Responsibilities

Conduct comprehensive psychosocial assessments for patients in need of mental health evaluation
Coordinate care plans with interdisciplinary teams to ensure patient well-being
Provide crisis intervention and support for individuals requiring involuntary psychiatric holds (5150)
Advocate for patients' rights and access to community resources
Document all interactions and interventions accurately and timely
Collaborate with Dignity Health facilities to ensure continuity of care

Qualifications

Current and active LCSW license in California
Valid BLS certification
5150 certification completed (training and exam with 80% pass rate)
Prior experience with Dignity Health systems is required
Two supervisory references (charge accepted) – one within 12 months and one within 3 years
Cannot have been directly employed by CommonSpirit/CHI/Dignity in the past year

Knowledge & Skills

In-depth knowledge of California mental health laws and 5150 procedures
Strong assessment and crisis intervention skills
Excellent communication and documentation abilities
Ability to work collaboratively in a fast-paced care coordination environment
Proficiency in electronic health records
