Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Perform comprehensive physical therapy evaluations and develop individualized treatment plans
Conduct start of care assessments, recertifications, and resumption of care visits
Provide skilled therapy services in patients' homes, focusing on functional independence
Collaborate with interdisciplinary team including clinical coordinators and sister agencies
Complete OASIS documentation and maintain accurate patient records

Qualifications

Active North Dakota Physical Therapy license
Current BLS certification
Valid Driver's License and auto insurance
Home health experience strongly preferred
OASIS training and experience preferred
Ability to work independently and manage a caseload of 5-6 visits per day

Knowledge & Skills

In-depth knowledge of OASIS documentation and home health regulations
Strong clinical assessment and problem-solving skills
Excellent communication and patient education abilities
Time management and organizational skills to handle daily visits and admissions
Comfortable using electronic health records and scheduling systems
