Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Provide physical therapy services to patients in their homes, promoting independence and quality of life
Develop and implement individualized care plans based on patient assessments
Collaborate with interdisciplinary care team to ensure coordinated care
Educate patients and families on therapeutic exercises and home safety
Document patient progress and maintain accurate clinical records

Qualifications

Active California Physical Therapist license required
Current BLS certification from the American Heart Association
Minimum 1 year of recent home health experience preferred
Excellent communication and interpersonal skills

Knowledge & Skills

Home health and community-based care best practices
Comprehensive knowledge of physical therapy modalities and interventions
Strong clinical assessment and treatment planning skills
Proficiency in electronic medical records (EMR) systems
