Contract Type: Travel Contract

Shift Type: Days, Monday-Friday, with no weekends or holidays required

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct initial evaluations and start-of-care visits for orthopedic and geriatric patients
Develop and implement individualized treatment plans to improve mobility and function
Provide skilled physical therapy services in patients' homes across Tehama County
Collaborate with nursing and therapy team to ensure coordinated patient care
Document patient progress in HCHB (HomeCare HomeBase) EMR system

Qualifications

Active Physical Therapy license in California (or compact eligibility)
BLS certification required
Minimum 1 year of home health experience (new travelers with less than 2 years considered)
Ability to travel within service area (approx. 40 miles/day)

Knowledge & Skills

Proficiency in home health therapy and start-of-care processes
Experience with HCHB (HomeCare HomeBase) EMR
Knowledge of orthopedic and geriatric patient populations including CHF, COPD, joint replacements
Strong time management and organizational skills
