Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive Oasis Assessments and Start of Care evaluations for patients in their homes
Develop and implement individualized treatment plans for elderly patients recovering from falls and fractures
Provide skilled physical therapy interventions to improve mobility, strength, and independence
Collaborate with interdisciplinary team members to ensure coordinated patient care
Accurately document patient progress and treatment using a tablet-based system
Educate patients and families on home exercise programs and fall prevention strategies

Qualifications

Active Indiana Physical Therapist license required
Experience with Oasis Assessments and Start of Care preferred
Home health experience is a plus
Ability to travel to patient homes across four counties (Noble, Lagrange, Steuben, DeKalb)
Valid driver's license and reliable transportation

Knowledge & Skills

Proficiency in Oasis Assessment documentation
Knowledge of home health regulations and best practices
Strong clinical reasoning and problem-solving skills
Excellent communication and interpersonal skills
Comfortable using tablet-based time tracking and documentation systems
